Job Summary and Qualifications: As the Manager for Case Management Services, you will play a pivotal role in promoting patient-centered care. Reporting to the Case Management Director, your responsibilities will include coordinating all aspects of hospital-based case management, supervising a dedicated team, and ensuring the highest standards of care.

What You Will Do:

Be a role model and clinical resource for your team.
Oversee staff development and daily department activities.
Ensure staff competence in care coordination.
Collaborate closely with physicians, nurses, and the interdisciplinary care team.
Monitor quality interactions, documentation, and assessments.
Maintain compliance with policies, procedures, and quality standards.
Advocate for patient rights, needs, and values.

Qualifications:

BSN Required.
Registered Nurse.
3+ years of clinical nursing experience.
2+ years of case management experience.
InterQual experience is a plus.

Additional Details:

Take on administrative responsibilities for our Division Director of CM.
Work one day a week at SDMC and fill a float pool CM position in Austin.
Manage the CM float pool, a mix of Social Work and Case Management (11 FTE).
Opportunity for occasional travel to El Paso.
Competitive salary with relocation option.
