Job Summary and Qualifications: As the Manager for Case Management Services, you will play a pivotal role in promoting patient-centered care. Reporting to the Case Management Director, your responsibilities will include coordinating all aspects of hospital-based case management, supervising a dedicated team, and ensuring the highest standards of care.
What You Will Do:
- Be a role model and clinical resource for your team.
- Oversee staff development and daily department activities.
- Ensure staff competence in care coordination.
- Collaborate closely with physicians, nurses, and the interdisciplinary care team.
- Monitor quality interactions, documentation, and assessments.
- Maintain compliance with policies, procedures, and quality standards.
- Advocate for patient rights, needs, and values.
Qualifications:
- BSN Required.
- Registered Nurse.
- 3+ years of clinical nursing experience.
- 2+ years of case management experience.
- InterQual experience is a plus.
Additional Details:
- Take on administrative responsibilities for our Division Director of CM.
- Work one day a week at SDMC and fill a float pool CM position in Austin.
- Manage the CM float pool, a mix of Social Work and Case Management (11 FTE).
- Opportunity for occasional travel to El Paso.
