Job Title: Home Health Registered Nurse

Location: Santa Rosa, CA

We are currently seeking an experienced Home Health Registered Nurse to join our team on a 13-week renewable contract basis. In this role, you will be responsible for providing exceptional patient care in the comfort of the patient's home.

Responsibilities:

Provide high-quality nursing care to patients in their homes
Perform assessments, develop care plans, and coordinate with other healthcare professionals to ensure optimal patient care
Administer medications and treatments as ordered by physicians
Educate patients and their families on disease management, medication regimens, and self-care techniques
Document patient care and other relevant information accurately and timely

Requirements:

Current RN license in California
Minimum of 1 year of Home Health nursing experience
BLS certification
Excellent communication and interpersonal skills
Ability to work independently and manage a patient load efficiently
