Salary: 110,000−110,000−130,000 per year (based on experience)
Sign-On Bonus: $10,000
Relocation Assistance: Available on a case-by-case basis
Performance Bonus: Up to 15% of annual salary
Position Overview:
We are seeking an experienced and visionary Director of Case Management to lead our team. This role offers a competitive salary, a $10,000 sign-on bonus, and the opportunity to earn a 15% performance-based bonus. Relocation assistance is also available for qualified candidates. As the Director of Case Management, you will oversee departmental operations, ensure compliance with policies, and drive performance improvement initiatives to optimize patient care and resource utilization.
Key Responsibilities:
Lead and evaluate departmental operations, including staffing, technology use, and onboarding processes.
Ensure compliance with care coordination, discharge planning, and documentation standards.
Collaborate with CFO and Division leaders to achieve departmental and organizational goals.
Analyze data and evaluate processes to drive sustainable performance improvement.
Foster strong relationships with internal and external stakeholders, including clinical and administrative leadership.
Ensure adherence to organizational policies, SOPs, and the company’s "Code of Conduct."
Qualifications:
Required:
Active RN licensure.
Minimum of 2 years of recent large acute care case management leadership experience.
Bachelor’s degree in Nursing or a related field.
Preferred:
Master’s degree in Nursing, Health Administration, or Business Administration.
Proven experience working in a complex healthcare environment, including engagement with C-suite leaders.
