Job Title: Corporate Director of Case Management - Nashville

  • Salary Range: $109,400  - $136,700 

Location: Nashville, TN

Position Type: Full-time, Permanent

Overview: We are seeking a Corporate Director of Case Management. In this role, you will lead various aspects of Case Management, including Transitional Care, Post-Acute Services support, Social Work programs, and Case Management-related Regulatory Standards. You will collaborate with cross-functional teams to develop and implement strategic objectives, identify improvement opportunities, and standardize processes to enhance outcomes and efficiency.

Key Responsibilities:

  • Evaluate facility practices and industry changes to ensure up-to-date Case Management processes.
  • Conduct needs analyses, identify gaps, and develop program components to address them.
  • Implement evidence-based toolkits to support Case Management initiatives.
  • Create and evaluate the strategic plan for clinical aspects of case management.
  • Develop data priorities and report formats to display critical factors.
  • Ensure compliance with governmental and regulatory agencies.
  • Provide feedback on data reporting to Case Management Leadership and other clinical areas.
  • Collaborate with related departments to promote and achieve Case Management goals.
  • Develop, implement, evaluate, and revise Case Management programs for optimal patient outcomes.
  • Direct the integration of clinical documentation and electronic health record technology.
  • Develop Care Coordination education and training on best practices and regulatory compliance.
  • Direct the development of Discharge Planning tools to support Case Management initiatives.

Education/Experience Requirements:

  • Bachelor's degree in Nursing or licensed social worker (LCSW).
  • Master's degree preferred.
  • 5+ years of Case Management experience required.
  • 3+ years of Case Management leadership experience required.
  • Performance Improvement experience required.
  • Strong oral, written, and presentation skills.

Certification/Licensure Requirements:

  • TN RN/LCSW State License.
  • Basic Cardiac Life Support (BLS).

Additional Details:

  • Shifts: Days, no direct reports.
  • Salary Range: $109,400 (minimum) - $136,700 (maximum), based on years of experience.
  • Up To 15% annual bonus, based on performance KPIs.
  • Relocation assistance on a case-by-case basis.
