Case Manager RN – Full-Time | Gainesville, FL
Day Shift: 8:00 AM – 4:30 PM (Rotating Weekends)
Evening Shift: 12:00 PM – 8:30 PM or 11:00 AM – 7:30 PM (Mon–Fri, No Weekends)
Hourly Pay Range:
$32.91/hr (min) – $45.25/hr (max based on experience)
Evening & Weekend Differentials: +$2.00/hr
Sign-On Bonus: $15,000
Relocation Assistance: Case by case
About the Role
We are hiring a Registered Nurse Case Manager who is driven, organized, and passionate about ensuring patients receive the right care at the right time. You will guide patients through the continuum of care, reduce avoidable delays, and serve as a liaison across multidisciplinary teams to ensure safe, timely discharge and optimal health outcomes.
This is not a desk job. This is an on-your-feet, fast-paced role that involves direct care coordination, patient and family interaction, and consistent engagement with medical teams.
What You’ll Do-Responsibilities
Perform clinical assessments and ensure appropriateness of care level
Coordinate admissions, pre-op processes, and discharge planning
Communicate with payors to ensure authorization and avoid denials
Conduct utilization reviews using InterQual or similar criteria
Collaborate with physicians, nursing staff, social workers, and therapists
Educate patients and families about treatment plans and post-discharge care
Maintain accurate documentation and reports using MIDAS
Participate in interdisciplinary team (IDT) rounds and quality improvement efforts
Requirements
Active Florida RN license (or compact RN with FL license within 60 days)
3+ years of recent acute care RN experience required
Preferred: Prior RN case management or utilization review experience
Will consider strong ICU or Med-Surg background or LTAC case managers
BLS (AHA) required within 30 days
BSN preferred
Strong multitasking, problem-solving, and communication skills
Certification in Case Management (CCM or ACM) preferred
