Location: Tallahassee, FL
Schedule: Full-Time – Day Shift (8:00 AM–4:30 PM) or Evening Shift (12:00 PM–8:30 PM)
Weekend Requirement: 1 weekend per 4-week schedule
Pay Range: $31.94 – $43.92 per hour (based on experience)
Differentials: $2.50/hr for evenings | $2.00/hr for weekends | Stackable: $4.50/hr if working both
Sign-On Bonus: $10,000 paid in 5 installments over 2 years
Relocation Assistance: Case-by-case basis

Overview:

Join the dynamic case management team at a 288-bed acute-care hospital serving Leon and Gadsden Counties. As a Case Manager RN, you will facilitate high-quality patient care by coordinating interdisciplinary treatment plans and discharge processes to ensure efficient resource utilization and excellent patient outcomes.

Responsibilities:

Perform assessments of psychosocial and medical needs
Develop and coordinate interdisciplinary care plans
Lead and participate in case reviews and team meetings
Monitor medical necessity and admission criteria
Communicate with third-party payers and facilitate transitions of care
Advocate for patients across the continuum of care
Identify care barriers and develop action plans for improvement

Requirements:

Minimum 3 years of recent Case Manager experience in an acute care setting
(OR)
3 years of combined experience in Med/Surg, Tele, ICU, PCU, Neuro, or ED
(OR)
Home Health/Insurance case management experience with at least 1 year acute care experience within the last 5 years
Associate Degree or Diploma in Nursing required (BSN preferred)
Active Florida RN license or Compact RN license with FL endorsement within 90 days
Case Management certification preferred
