Location: 320 N Crawford Street, Willows, CA 95988
Shift: 8:00 AM - 4:30 PM

Job Summary:
Willows Post Acute, a small, homelike skilled nursing facility, is seeking a passionate Physical Therapist to join our dedicated team. This is an excellent opportunity to contribute to patient recovery and rehabilitation in a nurturing and supportive environment.

Qualifications:

Valid PT license in California
Experience in a skilled nursing facility is preferred
Strong commitment to providing patient-centered care

Responsibilities:

Evaluate patient physical therapy needs and develop individualized treatment plans
Implement therapy interventions to improve patient mobility and functional abilities
Collaborate with a multidisciplinary team for comprehensive patient care
Document patient progress and therapy outcomes
