Manager of Case Management – $142,771–$214,032/year
Location: Riverside, CA
Schedule: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM (Rotating Weekends, Flexibility Required)

Position Overview
A large, high-volume acute care hospital is seeking a skilled Manager of Case Management to lead a multidisciplinary team focused on efficient patient throughput, safe discharge planning, and regulatory compliance. The hospital supports over 500 beds and provides services across multiple inpatient units.

Key Responsibilities

• Supervise daily activities of 15–18 RN Case Managers and Assistants

• Manage staffing, discharges, transitions of care, and throughput operations

• Conduct rounds and provide mentorship and coaching to staff

• Troubleshoot delays in care and escalate systemic barriers

• Coordinate with payers and post-acute care providers to support discharge planning

• Track case management performance metrics and ensure regulatory compliance

• Collaborate with the Director of Case Management to meet departmental goals

Qualifications

• Education: BSN required; Master’s degree preferred

• Licensure: Active California RN license (or eligibility/pending license)

• Experience:

  • Minimum 3–5 years of acute care case management or nursing leadership experience

  • Strong understanding of InterQual, M&R criteria, and payer systems

  • Familiarity with regulatory standards and discharge planning protocols
