Shift: 5x8-Hour (08:00 - 17:00)

Job Duration: 13-week renewable contract

Salary: $7,104-$8,288/month

We are currently seeking a skilled and compassionate Home Health Registered Nurse to join our team on a full-time basis. As a Home Health RN, you will provide high-quality nursing care to patients in the comfort of their own homes. This position offers a day shift schedule and a 13-week renewable contract.

Responsibilities:

• Conduct initial assessments and ongoing evaluations of patients' health conditions, including physical, psychological, and social aspects.
• Develop and implement individualized care plans based on patients' needs and goals.
• Provide skilled nursing care, including wound care, medication management, and monitoring vital signs.
• Collaborate with physicians, therapists, and other healthcare professionals to coordinate and ensure comprehensive patient care.
• Educate patients and their families on managing their conditions, medications, and home healthcare procedures.
• Regularly communicate with the interdisciplinary team regarding patients' progress, challenges, and updates.
• Maintain accurate and up-to-date documentation of patient assessments, interventions, and outcomes using the required Home Care Home Base (HCHB) charting system.
• Adhere to all regulatory guidelines, professional standards, and best practices in home healthcare.
• Maintain patient privacy, confidentiality, and dignity throughout all interactions and care delivery.

Requirements:

• Active and unrestricted Tennessee RN license or compact license in good standing.
• Current BLS certification.
• Minimum of 1 year of experience as an RN in home healthcare, with specific experience in wound care.
• Proficiency in HCHB charting or willingness to learn and adapt to the system.
• Excellent communication and interpersonal skills to effectively interact with patients, families, and the healthcare team.
• Valid driver's license and reliable transportation.
• Mileage reimbursement at IRS standard rates.
• Compliance with COVID vaccination status and booster requirements.
