Case Manager RN – Full-Time
Location: Gainesville, FL
Shifts Available:

  • Day Shift: 8:00 AM – 4:30 PM (Rotating Weekends)

  • Evening Shift: 12:00 PM – 8:30 PM or 11:00 AM – 7:30 PM (Monday–Friday, No Weekends)


Pay & Incentives

  • Hourly Rate: $32.91/hr – $45.25/hr (commensurate with experience)

  • Shift Differentials: +$2.00/hr Evenings | +$2.00/hr Weekends

  • Sign-On Bonus: Up to $15,000

  • Relocation Assistance: Available (case by case)


About the Role

We are seeking a results-driven Case Manager RN with recent acute care or LTAC experience to coordinate discharge planning, utilization review, and care transitions in a fast-paced hospital setting. This is an ideal opportunity for RNs with strong clinical backgrounds (e.g., ICU, Med-Surg) who excel at critical thinking, communication, and teamwork.


Key Responsibilities

  • Conduct clinical assessments and determine appropriate level of care and admission

  • Lead discharge planning and coordinate transitions across the care continuum

  • Perform utilization review using InterQual or other evidence-based tools

  • Partner with physicians to evaluate patient progression and adjust care plans

  • Provide updates on insurance status and discuss potential denials with patients/families

  • Support surgery pre-admission processes and collaborate with provider offices

  • Maintain accurate documentation in MIDAS and electronic health records

  • Participate in interdisciplinary team (IDT) rounds and escalate care barriers

  • Collaborate with social workers, payors, and the physician advisor

  • Educate patients and families on discharge planning, benefits, and resources

  • Assist with payor denial management and Health Insurance Notifications (HINNs)

  • Stay up to date on CMS guidelines, managed care contracts, and compliance protocols


Qualifications

  • 1+ year of recent acute care RN Case Management experience required

    • OR 2–3 years of recent ICU or strong Med-Surg RN experience

    • LTAC case management experience considered

  • Active Florida RN license or Compact RN license (must obtain FL license within 60 days)

  • BLS (AHA) required within 30 days of hire

  • Associate Degree in Nursing required; BSN preferred

  • Case Management, Utilization Review, or Nursing certification preferred

  • Proficiency in InterQual required; experience with MIDAS a plus

  • Strong communication, time management, and decision-making skills
