Job Summary:

As the Director of Case Management, you'll oversee department activities, ensure compliance, and coordinate patient care processes for optimal outcomes.

Responsibilities:

📊 Direct and evaluate departmental operations to achieve performance objectives.
🤝 Establish working relationships with key stakeholders for effective care coordination.
📋 Assess department performance and implement improvement activities.
🔍 Ensure compliance with policies, SOPs, and care coordination processes.

Qualifications:

🎓 Bachelor's degree required; Master's degree preferred.
💼 RN licensure or LCSW required.
🏥 5-7 years' experience in acute care Hospital Case Management.
👩‍💼 2+ years' experience in case management leadership preferred.
