Day Shift: 8:00 AM - 4:30 PM (Rotating Weekends)
Evening Shift: 12:00 PM - 8:30 PM or 11:00 AM - 7:30 PM (Mon-Fri, No Weekends)

Hourly Pay Range:

  • $32.91/hr (min) - $45.25/hr (max, based on experience)

Shift Differentials:

  • +$2.00/hr Evenings

  • +$2.00/hr Weekends

Sign-On Bonus: Up to $15,000
Relocation Assistance: Case by case

About the Role:

We are hiring a results-driven Case Manager RN with recent acute care or LTAC experience to lead discharge planning, utilization review, and patient care coordination in a fast-paced hospital environment. This position is ideal for RNs with strong clinical backgrounds (e.g., ICU, Med-Surg) who can communicate effectively, think critically, and adapt quickly to dynamic conditions.

Responsibilities:

  • Conduct clinical assessments and determine level of care and appropriateness of admissions

  • Coordinate interdisciplinary discharge planning and facilitate care transitions

  • Perform utilization reviews using InterQual or other decision-support tools

  • Work closely with physicians to evaluate status changes and develop/adjust care plans

  • Communicate insurance updates and potential denials to physicians, patients, and families

  • Support pre-admission and surgery preparation processes with physicians' offices

  • Maintain detailed, up-to-date case notes in MIDAS and other systems

  • Participate in daily IDT rounds and escalate delays or barriers in patient progression

  • Collaborate with ancillary teams, social workers, and the physician advisor for care optimization

  • Educate patients/families on discharge plans, benefits, and post-care instructions

  • Assist with management of payor denials and issuance of HINNS

  • Maintain knowledge of CMS guidelines, managed care contracts, and hospital policies

Requirements:

  • 1+ year of recent acute care RN case management experience required (Or 2-3 years of recent ICU or strong Med-Surg RN experience considered)

  • LTAC case management experience also considered

  • Active Florida RN license or Compact license (must obtain FL within 60 days)

  • BLS (AHA) required within 30 days of hire

  • Associate Degree in Nursing required; BSN preferred

  • Certification in Case Management, Utilization Review, or Nursing preferred

  • Proficiency in InterQual required; MIDAS experience a plus
