Job Description:

As a Full-Time Home Health LVN you will play a crucial role in providing exceptional healthcare services to patients in their homes. You'll have the opportunity to develop strong patient relationships and deliver care that promotes independence and overall well-being.

🔍 Responsibilities:

Conduct in-home patient assessments and develop individualized care plans.
Administer medications, perform wound care, and assist with various medical procedures.
Monitor patient vital signs and document changes in health status.
Educate patients and their families on proper self-care techniques.
Collaborate with the healthcare team to ensure a coordinated and holistic approach to patient care.
Maintain accurate and up-to-date patient records.
Provide emotional support and companionship to patients.
Adhere to all applicable laws, regulations, and company policies.

📋 Requirements:

Current LVN licensure in the state of Texas.
Minimum of 1-2 years of experience as an LVN, preferably in home health.
Strong clinical and assessment skills.
Excellent communication and interpersonal abilities.
Reliable transportation and a valid driver's license.
Ability to work independently and as part of a team.
