Shift: 5x8-Hour (08:00 - 17:00)

Job Duration: 13-week Renewable Contract

Salary: $7,104-$8,288/month

Are you a compassionate and experienced Registered Nurse with a passion for providing quality care in a home health setting? We have an exciting opportunity for you to join our team in Wasilla, AK, as a Home Health RN. As a Home Health Nurse, you will play a crucial role in delivering comprehensive healthcare services to patients in the comfort of their own homes.

Responsibilities:

Provide skilled nursing care to patients in their homes, ensuring the delivery of high-quality and individualized care.
Conduct initial assessments and develop personalized care plans for each patient, in collaboration with the interdisciplinary team.
Perform ongoing assessments, including monitoring vital signs, medication management, wound care, and symptom management.
Administer medications, IV therapy, and treatments as prescribed by physicians.
Educate patients and their families on disease management, medication administration, and self-care techniques.
Monitor and document patients' progress, maintain accurate medical records, and communicate any changes or concerns to the healthcare team.
Coordinate and collaborate with other healthcare professionals involved in the patient's care, such as therapists, social workers, and physicians.
Ensure compliance with regulatory guidelines, home health policies, and procedures.
Utilize Alora software for documentation and care coordination.

Requirements:

Valid Registered Nurse license in the state of Alaska (or eligible to obtain).
Minimum of 1 year of experience in Home Health nursing.
Proficiency in conducting Oasis assessments and documenting Oasis data.
Experience working with Alora software preferred.
Excellent assessment, communication, and interpersonal skills.
Ability to work autonomously and manage time effectively.
Point Expectation of 30 or higher, demonstrating productivity and efficiency.
Current COVID-19 vaccination or eligibility for a medical/religious exemption as per facility guidelines.
