Social Worker – Full-Time | Days | Fort Walton Beach, FL

Shift: Days with rotating weekends (8:00 AM – 4:30 PM; may be 9:00 AM – 5:30 PM or 9:30 AM – 6:00 PM)
Pay Range: $24.93 – $33.59/hr (based on experience)
Relocation Assistance: Available, case by case

We are seeking a compassionate and skilled Licensed Clinical Social Worker (LCSW) to join our interdisciplinary care team at HCA Florida Fort Walton-Destin Hospital, a 267-bed acute care facility. In this role, you will play a key part in discharge planning, crisis intervention, and patient/family support across inpatient and emergency department settings.

Responsibilities

Conduct biopsychosocial assessments and develop discharge plans for high-risk patients
Identify and address barriers to recovery, including financial, emotional, psychosocial, and spiritual challenges
Provide crisis intervention, including support for end-of-life, trauma, and terminal illness cases
Collaborate with case managers, physicians, nurses, payers, and interdisciplinary staff to ensure safe and timely discharge planning
Educate patients and families on available community and healthcare resources
Maintain strong working knowledge of Medicare, Medicaid, and private insurance rules and managed care concepts

Qualifications

Master’s Degree in Social Work (MSW) required
Active Florida LCSW required (endorsement accepted if eligible)
At least 1 year of recent (within last 3–4 years) acute care hospital social work experience OR 5+ years of prior hospital experience if not recent
Discharge planning experience required
Experience with PASSRs (Preadmission Screening and Resident Review) and ESPERs required
Strong organizational, multitasking, and critical thinking skills
Hospital, insurance company, or county social services experience preferred
