📋 Responsibilities:

Conduct in-home patient assessments to evaluate physical condition.
Develop and implement personalized treatment plans.
Administer physical therapy interventions and monitor progress.
Collaborate with a multidisciplinary healthcare team.
Educate patients on therapy goals and home exercises.
Maintain accurate patient records and documentation.
Contribute to the overall well-being and recovery of patients.

📝 Qualifications:

Valid Physical Therapist license in the state of Texas.
Master's or Doctorate degree in Physical Therapy.
Previous experience in a clinical or home health setting preferred.
Strong clinical and interpersonal skills.
Ability to work independently and provide exceptional care in a home setting.
